چکیده

Single maintenance and reliever therapy The paper by Chapman et al reviewing single maintenance and reliever therapy (SMART) in asthma is important in highlighting some of the inadequacies of existing research evaluating this treatment method. The authors also claim that SMART fails to achieve adequate asthma control as measured by GINA criteria and provide a table detailing seven studies and associated control indices. While these outcomes are far from ideal, the authors fail to point out that they were no worse than the comparator arm, which varied across the studies from conventional inhaled steroid therapy to fixed dose combination inhaled steroid/long-acting betaagonist inhalers in high dose (ie, ‘optimal therapy’). This inadequate control therefore reflects the severity of disease in the trial subject group rather than being a specific deficiency of SMART therapy. It is disingenuous to claimthatSMARTfails to achieve adequate asthma control without pointing out that in this patient group standard, ‘optimal’, therapy does no better.

The effect of budesonide/formoterol maintenance and reliever therapy on the risk of severe asthma exacerbations following episodes of high reliever use: an exploratory analysis of two randomised, controlled studies with comparisons to standard therapy

BACKGROUND Divergent strategies have emerged for the management of severe asthma. One strategy utilises high and fixed doses of maintenance treatment, usually inhaled corticosteroid/long-acting β2-agonist (ICS/LABA), supplemented by a short-acting β2-agonist (SABA) as needed. Alternatively, budesonide/formoterol is used as both maintenance and reliever therapy. The latter is superior to fixed-d...

Budesonide/formoterol maintenance and reliever therapy: impact on airway inflammation in asthma.

The aim of the present study was to compare the effectiveness, safety and health economics of budesonide/formoterol maintenance and a novel reliever therapy with conventional best practice in patients with persistent asthma in Canada. After 2 weeks of usual therapy, 1,538 patients were randomised for 6 months to open-label budesonide/formoterol maintenance and reliever therapy 160/4.5 microg tw...

Overall asthma control achieved with budesonide/formoterol maintenance and reliever therapy for patients on different treatment steps

BACKGROUND Adjusting medication for uncontrolled asthma involves selecting one of several options from the same or a higher treatment step outlined in asthma guidelines. We examined the relative benefit of introducing budesonide/formoterol (BUD/FORM) maintenance and reliever therapy (Symbicort SMART® Turbuhaler®) in patients previously prescribed treatments from Global Initiative for Asthma (GI...

Description of the protocol for the PRACTICAL study: a randomised controlled trial of the efficacy and safety of ICS/LABA reliever therapy in asthma

INTRODUCTION In adult asthma, combination inhaled corticosteroid (ICS)/fast-onset long-acting beta agonist (LABA) used solely as reliever therapy may represent an effective and safe alternative to ICS maintenance and short-acting beta agonist (SABA) reliever therapy. OBJECTIVE To compare the efficacy and safety of ICS/fast-onset LABA reliever therapy with ICS maintenance and SABA reliever the...

Satisfaction levels and asthma control amongst Malaysian asthmatic patients on budesonide/formoterol maintenance and reliever therapy: experience in a real-life setting

BACKGROUND Budesonide/formoterol used for both maintenance and reliever therapy has been shown to benefit patients with persistent asthma. We evaluated patient satisfaction and asthma control among Malaysian patients prescribed budesonide/formoterol as single maintenance and reliever therapy in a real-life clinical practice. METHODS Adult patients diagnosed with partially controlled or uncont...
